The right differentiation of patients based on the degree of their disorders’ risk is an essential precondition for the appropriate use of secondary prevention. In fact, 75 per cent of patients’ disorders classify as low risk. Such cases can be treated according to the principles of general metaphylaxis. Only 25 per cent of patients face a high risk of further attacks. Consequently, it is imperative to identify these cases in time and perform additional metabolic tests. Having correctly identified the individual biochemical risk in these patients, a specific metaphylaxis approach using suitable drugs must be taken.

Contemporary methods in diagnosis and treatment of premature ejaculation are discussed. Overview of partners and psychological aspects of the sexual dysfunction is given. Biological therapeutic methods include topical anesthetics and pharmacotherapy. Most frequently are in this indication used some serotoniergic antidepresives. Fournier gangrene is a very serious necrotic fasciitis of external genitals and perianal area. It mostly has an impact on people with weakened immunity and also with serious comorbidity disorder. It is often possible to find gateway to the infection. As a treatment there is used a radical surgery by removing necrotic substance, broad-spectrum antibiotics, derivation of urine and observation at intensive care unit. According to our case study a morbidity and mortality of this disease is rather high and ranged between 20–75 % despite the early intervention in the treatment and a progress in therapeutic access.
