The predominant urologic problems in geriatric patients are symptoms of the lower urinary tract. Most include incontinence, detrusor instability or benign prostatic hyperplasia. Daily functioning of older patients is affected by these problems, reducing the quality of his life. Help is often difficult. The solution could be pharmacotherapy. However, due to the use of many drugs to other health problems, there is a high probability of side effects. Therefore, the need of each patient individually assessed and proceeds very cautiously.

To evaluate lower urinary tract symptoms, it is important to utilize objective methods since subjective description of symptoms need not completely reflect the reality and may differ significantly in each patient (1, 2, 3). Thus, investigation methods to obtain objective results are used in diagnosing lower urinary tract symptoms (4, 5, 6). Uroflowmetry is among the basic urodynamic methods and its main benefits are non-invasiveness, simple use, easy reproducibility, and possibility of repetition over time.

The premature or rapid ejaculation is a second most common sexual dysfunction. It creates difficulties for our patients as well as for their partners. The treatment was not very successful until the recent time. With new psychofarmacological drugs, especially with new antidepressants, are our possibilities much better. The authors provide an overview of the possible therapeutical approaches. The therapeut can apply biological treatment or psychotherapy. The combination of both, with an accent on pharmacotherapy, may be the most successful.

Malakoplakia of the bladder as an unusual reason of bilateral obstruction

Malakoplakia is a rare type of the chronic infection of the urinary tract caused by atypical immunity reaction. We present a case of patient with bilateral hydronephrosis and renal insufficiency, with cystoscopic finding of unspecific tumor-like focuses in the bladder as the cause of the obstruction of both ureters. Malakoplakia was found by histological investigation of the resected tissue. The article summarizes basic diagnostic and therapeutic procedures in this uncommon urological disease.
