One of the most important adverse effects of antibiotics is the increasing resistance of bacteria to their activity. Rational antibiotic therapy is therefore an integral part of preventing bacterial resistance from developing. Nosocomial urinary tract infections are caused by numerous bacterial species, often with higher levels of resistance to antimicrobial agents. Aminopenicillins or aminopenicillins with inhibitors of bacterial ß-lactamases combined with aminoglycosides can be used as drugs of first choice. However, it must be stressed that the regimens of initial antibiotic therapy are strongly influenced by local conditions reflected by the results of local bacterial surveillance.
