Metabolické vyšetření urolitiázy - význam, vyšetření, terapie a metafylaxe

Metabolické odchylky, odpovědné za recidivy urolitiázy, zjišťujeme u 97 % vyšetřených pacientů. Z této skupiny je až v 80% dosaženo remise. U většiny pacientů je příčinou vzniku močového konkrementu vysoký odpad kyselých radikálů, pocházejících z diety. Acidifikace moči je obvykle doprovázena hyperkalciurií. Příslušná úprava diety vede k omezení recidiv konkrementů, snížení ztrát kalcia a tím snížení rizika vzniku osteoporózy. Podle nových poznatků se na vzniku kalcium­oxalátové litiázy podílí také dekolonizace střevní flory. Ve zdravé střevní floře byla prokázány bakterie štěpící oxaláty. Tato skutečnost otvírá další možnosti prevence urolitiázy.

METABOLIC MANAGEMENT OF UROLITHIASIS - SIGNIFICANCE, EXAMINATION, THERAPY AND METAPHYLAXIS

Metabolic abnormalities responsible for stone reccurence are currently identified in 97 % of evaluated patiens and remission rates in this group is achieved in almost 80%. Urolithiasis secondary of urinary calcium excretion in most renal stone formers is more dependent on the dietary acid load. Reducing the acid-ash content of the diet has an impact on decreasing stone reccurence while preventing bone loss. New evidence associated the decolonization of oxalate degrading intestinal flora with a higher risk of calcium oxalate stone formation, possibly opening a novel approach for prevention of urolithiasis.
