Význam substituce androgeny u mužů s testikulární hypofunkcí

V průběhu intrauterinního vývoje a života jedince mužského pohlaví se setkáváme s testikulární hypofunkcí, která rezultuje v několik různých klinických obrazů. Vedle intrauterinní dysfunkce, která je provázena patologií funkce Sertoliho i Leydigových buněk a testikulární dysfunkcí v pubertě (trauma, chromozomální aberace, retence), je v posledních letech diskutována problematika tzv. parciálního deficitu androgenů, který snad provází každého stárnoucího muže. Hovoříme o andropauze s vědomím, že diagnostika je velice komplikovaná a terapie (s ohledem na možnou indukci adenokarcinomu prostaty) rovněž. Na začátku diskuze o andropauze však musí být vědomí komplexního efektu androgenů. O nutnosti substituce androgenů u mužů se ztrátou varlat není pochyb, není však stanoven věk, kdy tuto terapii ukončit.

Androgen substitution significance in men with testicular hypofunction

During intrauterine development and further on during a life of male individual we can encounter a testicular hypofunction, which results in several different clinical pictures. Besides intrauterine testicular dysfunction, which is accompanied by Sertoli and Leydig cells pathology and testicular dysfunction in puberty (trauma, chromosomal aberration, retention), so called -partial androgen deficiency-, which accompanies all aging men, is discussed in recent years. We speak of andropause (willing to approximate this situation to menopause in women) having on mind complicated diagnostic and therapy (regarding possible adenocarcinoma of prostate induction) either. But at the beginning of discussion about andropause complex androgen effect in men (and women) must be kept in mind. There is no need to doubt about androgen substitution in men with loss of testicles (bilateral tumour etc.). But an age, when therapy should be ended, is not stated.
