Naše zkušenosti s diagnostikou a léčbou primárního obstrukčního megaureteru

Pre- a postnatálním ultrazvukovým (UZ) screeningem se uspíšila diagnostika primárního obstrukčního megaureteru, který byl dříve diagnostikován až na základě klinických projevů. V letech 1987-2001 jsme operovali 42 pacientů se 49 primárními obstrukčními megauretery. Prenatálně byl diagnostikován u čtyř nemocných, u zbývajících byla diagnóza stanovena průměrně v 16. měsíci věku, u 26% dětí byla přítomna klinická symptomatologie. Indikace k operaci byla stanovena na základě UZ vyšetření, MCUG, VU a diuretické scintigrafie. Ureterocystoneostomie byla provedena u nemocných se signifikantní poruchou drenáže, s poklesem funkce a při symptomatické IMC. Průměrný věk operovaných byl 20 měsíců. Po operaci jsou všechny děti bez klinických obtíží, ledvinná funkce je stabilizována nebo zlepšena a při diuretické scintigrafii došlo k úpravě drenáže. Ultrazvukový screening umožnil včasnou diagnózou s určením správného léčebného postupu maximální ochranu funkce ledviny.
